Drawing from years of dealing with chronic injuries, he focuses on two main issues: a long-term lower back problem and more recent knee pain.

    For his back, Brad explains he’s been managing the effects of a herniated disc and multiple surgeries for nearly eight years, getting himself to about “93%” recovery through consistent routines and experimentation. Seeking that final improvement, he turns to AI for new ideas—specifically exploring sleep positioning, which he believes is the biggest trigger for his pain. AI recommends a “zero gravity” sleeping setup to reduce spinal pressure. Instead of investing in an expensive adjustable bed, Brad improvises with affordable wedges and pillows. The result is a noticeable improvement, bringing him close to “98%” recovery, making this a clear success story where AI offered a novel, effective solution he hadn’t tried before.

    He then tests AI on a different issue—chronic knee pain. The AI suggests muscle imbalances, particularly involving the VMO (inner quad), and provides exercises targeting strength, stability, and mobility. While some of these recommendations align with his training style and initially help, his progress stalls and one knee worsens. Eventually, Brad consults a physical therapist, who identifies tightness in his IT band and quads as key contributors—something the AI did not emphasize. With targeted treatment like foam rolling and specific strengthening exercises, his knee pain resolves within weeks.

    Brad concludes that AI can be a valuable, low-risk tool for exploring ideas, learning, and improving routines—especially for those with experience in fitness. However, it still falls short of replacing hands-on expertise and nuanced diagnosis from professionals. He emphasizes that better input and more detailed feedback to AI could improve its usefulness, but ultimately sees it as a supplement rather than a substitute for qualified care.

    In this episode of Over 40 Fitness Hacks, I sit down with Samuel Shepherd—a biochemical engineer with a wild backstory spanning petrochemicals, defense research, and cutting-edge science—to discuss his personal journey from a terminal diagnosis to discovering the potential of astaxanthin.

    Samuel shares how he was diagnosed with polycythemia vera, a rare and life-threatening blood cancer, and given slim odds of survival. After years of traditional management with no real solution, he took matters into his own hands—leveraging his background in chemistry and access to advanced research tools to search for answers.
    His breakthrough came from a simple but powerful question: “What living things don’t get cancer?”

    After researching species like salmon, flamingos, sharks, elephants, and naked mole rats, he identified a common compound among them—astaxanthin, a potent antioxidant derived from algae.

    He began experimenting on himself, initially consuming algae-derived astaxanthin in small doses. Over time, he observed measurable improvements in his condition—eventually eliminating the need for ongoing blood treatments.

    We break down the science in a way that makes sense for everyday listeners:
    Aging & disease are heavily linked to free radical damage

    These free radicals contribute to inflammation, DNA damage, and chronic disease

    As we age (around 40–50), our natural antioxidant production declines

    In this episode of Over 40 Fitness Hacks, I sit down with Dr. John Osborne, preventive cardiologist and founder of Clear Cardio, to talk about the biggest threat to longevity: cardiovascular disease.

    While much of my show has historically focused on fat loss, muscle building, and aesthetics, this conversation shifts toward what truly determines lifespan — heart health. Dr. Osborne makes it clear: if you don’t have lifespan, healthspan doesn’t matter.

    Dr. Osborne explains that cardiovascular disease has been the leading cause of death for over 120 years — responsible for roughly 40–45% of deaths, even during COVID. Globally, around 20 million people die each year from heart disease.
    The problem? We typically detect it far too late.

    Traditional cardiology focuses on finding severe blockages — the equivalent of detecting stage 4 cancer. But plaque (atherosclerosis) develops silently over decades. In fact, 85% of heart attacks occur in people without severe blockages detected beforehand, due to sudden plaque rupture.
    Even more alarming:
    Half of men and two-thirds of women experience a fatal event as their first symptom.

    Most people have no warning signs.

    Dr. Osborne’s mission is early detection — what he calls the cardiac version of a colonoscopy.
    At Clear Cardio, they use:
    Advanced 640-slice Cardiac CT technology

    AI-enhanced imaging capable of detecting plaque the size of a period at the end of a sentence

    Personalized prevention plans based on decades of lipid and cardiovascular expertise

    According to Dr. Osborne:
    99% of people have detectable plaque

    Only about 1% are completely plaque-free

    Most people feel perfectly fine and have normal stress tests

    The difference is that this technology detects plaque long before symptoms appear — when it’s still reversible.
    Dr. Osborne emphasizes that we already have powerful, proven tools:
    Nutrition and exercise

    Blood pressure and blood sugar management

    Advanced lipid therapies

    Targeted medications when necessary

    The issue isn’t lack of treatment — it’s lack of early detection.
    Even those with strong genetic risk can reduce their risk by 50% with proper lifestyle habits. And for those who “picked the wrong ancestors,” there are still highly effective treatment options available.
    The cardiac CT scan also provides additional insights, including:
    Lung imaging

    Fatty liver detection

    Bone density

    Spine assessment

    All with extremely low radiation exposure — roughly equivalent to a chest X-ray, and far less than traditional nuclear stress tests.
    Dr. Osborne compares modern heart prevention to the early days of insulin in 1922 — a turning point that transformed diabetes care. He believes we’re at a similar tipping point for heart disease.

    His ultimate goal?
    To eliminate plaque-driven heart disease entirely — and put himself out of business.

    Clear Cardio is currently operating in Dallas and Chicago, expanding to Manhattan, Miami, and other major cities, with the long-term goal of reaching high-population areas nationwide.
